* Providing individual and group therapy to promote consumer well-being and empowerment
* Coordinating and ensuring consumer involvement in the Person-Centered Planning Process (PCP)
* Developing and implementing individual plans of service to meet consumer needs

In addition to your clinical expertise, you will also be responsible for:

* Communicating with internal and external stakeholders to ensure seamless care coordination
* Facilitating the coordination of consumer intakes, discharges, and transfers
* Collaborating with nursing staff to provide behavioral supports and accompany consumers to appointments

**Requirements:**

* Master's degree in Social Work, Psychology, or Counseling
* Possession of or eligibility for one of the following licenses: LMSW, LLMSW, LP, LLP, TLLP, LPC, or LLC
