About the Role

As a Hospice Case Manager at Corewell Health, you will play a vital role in providing compassionate end-of-life care to individuals with a life expectancy of six months or less. Our team is dedicated to delivering high-quality, patient-centered care in the comfort of their own homes, nursing facilities, or in the hospital.

Key Responsibilities
  • Assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet a patient's end-of-life care needs.
  • Collaborate with the interdisciplinary team to create an outcome-based plan of care, ensuring the changing needs of the patient are met while facilitating a symptom-free, peaceful death.
  • Under the direct supervision of the Nursing Manager, use clinical judgment skills to access and implement the treatment plan for patients.

Qualifications
  • Associates in Nursing Required.
  • Bachelors in Nursing Required within 5 years.
  • MI Registered Nurse (RN) Required.
  • AHA or ARC Basic Life Support (BLS) Required within 90 days.
  • Driver's License within the State of Michigan.
Work Environment

Our Hospice team is based at SITE - Continuing Care E Beltline - Grand Rapids, with a full-time schedule of 40 hours per week, Monday to Friday, with on-call weekends.
