About the Role

The Integrated Health Program Coordinator will be responsible for managing and overseeing grant-funded or contracted services, stakeholder relations, and informing program development for the integrated health program. This role will focus on empowering AHC's 8 member federally qualified health centers to support opportunities and address challenges around access to integrated healthcare services.

Key Responsibilities:

  • Manage and oversee grant-funded or contracted services
  • Cultivate partnerships with Integrated Behavioral Health (IBH) and pediatric providers/staff in clinics and school-based health centers
  • Facilitate peer network(s) and other cohort building activities for care coordinators and their supervisors at AHC member health centers
  • Contribute to development of presentations or other collateral regarding mental health and substance use disorder programming in AHC member FQHCs

Requirements:

  • Bachelor's degree required; Master's preferred
  • At least 5 years' experience in healthcare or combination of relevant education and work experience
  • Excellent soft skills and desire to build trusting relationships
  • Collaborative workstyle and ability to foster collaboration with external partners of diverse backgrounds and experiences

Salary Range:$77,563.20 to $93,412.80 per year
