Job Overview

We are seeking an experienced Program Manager to lead our integrated health services program at Community Health Center Network Inc. The successful candidate will have a proven track record in managing grant-funded programs, building partnerships with healthcare providers, and developing effective strategies to address health disparities.

About the Role

The Program Manager - Integrated Health Services will be responsible for overseeing the management of grant-funded programs, stakeholder relations, and informing program development for our integrated health services program. This role requires strong project management skills, excellent communication and collaboration abilities, and a passion for addressing health disparities.

Key Responsibilities
  • Management and Oversight of Grant-Funded Programs: Develop and implement strategies to ensure timely submission of reports to funders, pursue funding opportunities to sustain and evolve program implementation, and collaborate with finance staff to ensure timely issuance of payments to subcontractors.
  • Stakeholder Relations: Cultivate partnerships with healthcare providers, community-based organizations, and government agencies to support successful delivery of care coordination activities.
  • Program Development and Advocacy: Contribute to the development of presentations and other materials regarding mental health and substance use disorder programming, participate in peer networks facilitated by California Primary Care Association, and represent the organization at local, state, and county-wide meetings.
