About the Role

Crestwood Behavioral Health is seeking a dedicated and compassionate Service Coordinator (Mental Health Case Manager) to join our team. In this critical role, you will be responsible for assessing clients to identify individual problem areas necessitating treatment and completing thorough assessments.

You will also coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. As a key member of our team, you will work closely with nursing staff to ensure continuity of treatment efforts.

This is a full-time position with a schedule of 24.48—29 USD per hour.

Responsibilities
  • Assess clients to identify individual problem areas necessitating treatment and complete thorough assessments.
  • Coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
  • Work closely with nursing staff to ensure continuity of treatment efforts.
Qualifications

To be successful in this role, you should have:

  • A bachelor's degree in psychology, social work, or a field related to mental health.
  • Two years of full-time experience or part-time equivalent in a mental health setting.
  • Demonstrated knowledge of DSM-V and psychosocial rehabilitation and recovery principles.
  • Experience in working with healthcare and mental health regulatory agencies.
