Key Responsibilities
- Contribute to the formulation of personalized service plans for new residents and regularly review and update these plans based on evolving resident needs.
- Assist in conducting comprehensive assessments of residents.
- Maintain organized and up-to-date resident records, ensuring all necessary documentation is completed accurately.
- Support the admissions and discharge processes to guarantee seamless service coordination.
- Regularly review physician orders for precision and compliance.
- Act as a liaison among families, healthcare providers, and relevant agencies concerning residents' health and wellness matters.
- Exemplify teamwork and foster positive interactions with a nurturing and professional demeanor.
- Perform additional duties as required.
Qualifications
- Valid RMA or RN/LPN license.
- Current adult CPR certification.
- Strong customer service and interpersonal communication skills.
- Proficient in or willing to learn various computer applications.
- UAI/ISP certification or readiness to obtain certification within 60 days of employment.
