Resident Service Coordinator

At NHE Inc, we are seeking a highly skilled and compassionate Resident Service Coordinator to join our team. As a key member of our staff, you will play a vital role in providing exceptional service to our residents, empowering them to live independently and exercise their right to self-determination.
The Resident Service Coordinator will proactively educate and provide referrals and services to residents, including those who are frail and at-risk. You will work closely with community service providers to ensure that residents receive the most appropriate services to meet their needs.
Key Responsibilities:
- Conduct comprehensive, non-clinical assessments of residents for wellness and social needs.
- Help residents identify, access, and coordinate services, including personal care services.
- Monitor the receipt and follow-through of services, encouraging resident engagement and participation in their own care/services management.
- Develop and sustain partnerships with community-based supportive service providers and other stakeholders.
- Maintain an up-to-date resource directory with local service providers.
Requirements:
- Bachelor's degree in social service-related field or meet HUD required work experience.
- Knowledge of and training in elderly and disability services.
- Demonstrated working knowledge of local services for the elderly and people with disabilities.
- Two or three years of social service delivery experience.
