Case Manager RN

The Case Manager RN is responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes. This role requires a strong clinical knowledge, effective interpersonal communication, and critical thinking skills.
Key Responsibilities:- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, and caregiver.
- Promote consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Minimum of 3 years in nursing practice, with clinical knowledge, IT skills, and effective interpersonal communication.
- Ability to work autonomously, with innovative thinking, resourceful navigation of community resources, and team building and leadership skills.
- Preferred experience in nursing practice, care management, community or ambulatory care management, primary care clinical experience, and inpatient discharge planning work.
- Required education: Bachelor of Science in Nursing or bachelor's degree in a related field.
- Required license: Current licensure as a registered nurse with the North Carolina Board of Nursing.
- Preferred certification: Nationally recognized certification in Case Management, such as ACM, CCM, or ANA certification.
