Job Summary

We are seeking a highly motivated and dedicated Family Services Assistant to join our team at Plaza de la Raza Child Development Services. The successful candidate will be responsible for connecting families to essential health, nutrition, and mental health services, while also developing relationships with parents to encourage trust and respect.

Key Responsibilities
  • Recruit and enroll eligible children into our program, ensuring 100% enrollment.
  • Conduct family assessments to determine eligibility for program services.
  • Provide resources to families for emergency assistance or crisis intervention and follow-up.
  • Serve as a liaison between our program and community-based agencies to enhance services.
  • Engage families in the Family Partnership process to identify needs and strengths, and provide resources as needed.
  • Obtain signed permission slips for and ensure all health and nutrition screenings are completed.
  • Assist families in arranging appointments and follow-up visits as needed.
  • Follow up with families on requirements for well-childcare, developmental screenings, dental care, and immunizations.
  • Conduct hearing and vision screenings and height/weight measurements to facilitate appropriate referrals.
  • Initiate referrals for services aligned with our program's policies and procedures.
  • Establish and maintain direct communication between the family, site staff, and content area coordinators/specialists.
  • Monitor child attendance and initiate parent conferences to discuss frequent absences.
  • Input and monitor all relevant child and family data through our record-keeping system.
  • Ensure compliance with ERSEA through maintenance of an ERSEA file for each enrolled child.
  • Participate in parent orientation, interdisciplinary team meetings, child IEP/IFSP meetings, and monthly center meetings.
  • Participate in transition planning for children entering and exiting our program.
  • Submit monthly monitoring reports and corrective action to the supervisor.
Requirements
  • Associate degree or Family Development Credential (FDC) and a High School Diploma.
  • One year of experience in parent involvement or social service settings assisting economically and socially disadvantaged families.
  • Successful completion of an initial health examination and proof of absence of Tuberculosis.
  • Documentation of current immunizations against influenza, Measles, Mumps, & Rubella (MMR), and Pertussis (TDAP).
  • Fingerprint clearances from the Department of Justice (DOJ), the Federal Bureau of Investigation (FBI), and the Child Abuse Index.
  • Completed and signed statement of any pending and/or prior criminal arrests/charges/convictions related to child sexual abuse, child abuse and neglect, and/or violent felonies.
  • Verification of eligibility for employment in the United States of America.
  • A valid California driver's license and private transportation or ability to obtain transportation.
  • Bi-lingual in a language reflective of the population served (preferred).
