The Clinical Nurse Care Coordinator plays a vital role in delivering direct patient care, orchestrating comprehensive home health services, and ensuring meticulous documentation for clients in alignment with established Quality Standards and under the supervision of a Clinical Supervisor. This position primarily serves the designated county, with potential visits to adjacent counties as necessary.

Home visits are scheduled Monday through Friday, from 8 AM to 4:30 PM, with one weekend per month mandatory. There is no on-call duty required.

Key Responsibilities and Expectations
  1. Guarantee the safe and high-quality provision of home health services, which includes managing client caseloads, formulating care plans based on client assessments and evaluations, employing an 'interdisciplinary' approach, and ensuring all documentation adheres to quality standards.
  2. Complete all necessary evaluations and ensure documentation accurately reflects nursing care and interactions, maintaining clarity and timeliness in all records.
  3. Oversee the coordination of LPNs; provide mentorship and education regarding client care, collaborating with the Clinical Supervisor and Patient Care Coordinator to effectively manage patient care using a team-oriented approach.
  4. Supervise paraprofessional staff related to patient care; guide home health aides in executing assigned tasks per the care plan and relay pertinent information to the home health aide department.
  5. Adhere to all organizational policies and procedures, ensuring patient and employee confidentiality at all times, while demonstrating knowledge of infection control and compliance with all relevant regulations.
  6. Stay informed and knowledgeable about all organizational quality improvement standards.
  7. Continuously update clinical knowledge and skills.
Qualifications
  1. Possession of a current New York State registered professional nurse license (degree must be from an accredited nursing program). A BSN is preferred.
  2. Valid driver's license and auto insurance, along with a vehicle available for work-related duties.
  3. Self-motivated and organized, capable of working independently in a dynamic environment.
  4. Prior experience in home care or case management is advantageous.
  5. Proficient in computer skills, including familiarity with electronic medical records software, with the ability to adapt to new software.
  6. Exceptional verbal and written communication abilities.
Physical Demands
  • Ability to remain seated or standing at a desk for the majority of the workday.
  • Capability to perform physical tasks, including lifting and carrying weights, patient transfers, and mobility in various weather conditions.
  • Ability to drive in ALL weather conditions.
  • Visual and auditory capabilities sufficient for effective communication.
  • Frequent use of a laptop for data entry and information retrieval.
  • Exposure to Bloodborne Pathogens Category III - minimal contact with blood and body fluids.
