Position Summary:

The Community Resource Center at SAC Health is a pivotal department focused on mitigating health disparities in the San Bernardino area. This role requires a foundational understanding of the Social Determinants of Health and their effects on underserved populations. Responsibilities include organizing and managing resources and information for community members, leading CRC initiatives, and preparing monthly reports, including the quarterly newsletter. The position involves maintaining precise documentation, fostering community partnerships, and conducting educational classes.

Key Responsibilities:

  • Facilitate program delivery and ensure timely completion of assigned tasks according to departmental protocols.
  • Oversee case management for staff involved in social services, providing guidance during client intake and support for complex needs.
  • Mentor new team members through mock interviews and observational support during their probationary period.
  • Collaborate with the CRC Supervisor to identify and implement essential projects.
  • Integrate Social Determinants of Health considerations into client care and staff training.
  • Lead community-focused events and programs under the guidance of the CRC Manager, participating in all relevant SACHS activities.
  • Work collaboratively with interdisciplinary teams to address challenges and ensure project success.
  • Manage outreach initiatives and health fairs, representing the department at community meetings.
  • Perform additional duties as required by the official job description.

Qualifications:

  • Education: Bachelor’s Degree in Public Health, Social Work, Human Services, or a related field.
  • Licensure/Certification: Valid California driver’s license, good driving record, and current auto insurance are required. EPIC certification for the relevant module is also necessary.
  • Experience: A minimum of three years in clinical or case management roles is required, with experience in community education preferred. Bilingual proficiency in English and Spanish is advantageous.
  • Technical Skills: Proficiency in project management software, spreadsheets, and word processing tools is essential.
  • Interpersonal Skills: Strong analytical, problem-solving, and communication skills are required, along with the ability to work effectively in a team environment.
  • Work Eligibility: Must be authorized to work in the United States on a full-time basis without requiring sponsorship.
