Position Summary: The Nurse Care Coordinator plays a vital role within the Enhanced Care Management (ECM) team. This initiative, funded through California Advancing and Innovating Medi-Cal (CalAIM), focuses on a holistic approach to health management, ensuring that individuals receive tailored support to optimize their health outcomes. ECM services are designed to be both clinically appropriate and cost-effective alternatives to traditional state plan services. All potential ECM participants must be enrolled in the Inland Empire Health Plan (IEHP). Our outreach efforts aim to engage and enroll members in the ECM program, ultimately improving their social and health determinants.

Key Responsibilities:

  • Facilitate access to primary and behavioral health services as required.
  • Collaborate effectively with Titanium's interdisciplinary team to deliver high-quality care management.
  • Engage members through therapeutic interactions to foster healthy behaviors and support mental health interventions.
  • Coordinate care by linking resources and collaborating with community professionals to ensure comprehensive care for members.
  • Manage seamless transitions of care in accordance with IEHP guidelines following emergency room visits and hospitalizations.
  • Conduct regular follow-up visits to monitor member progress.
  • Provide care coordination through various communication methods, including phone calls and emails, while maintaining accurate records.
  • Assist the Behavioral Health Care Manager and Community Health Worker with assigned tasks.
  • Participate in case reviews and collaborate on member care issues with ECM team members.
  • Consult with Clinical Nurse Leaders or Behavioral Health Care Managers for clinical decision-making.
  • Utilize engagement strategies and motivational interviewing techniques to involve members in their care.
  • Deliver health education to members within the scope of practice.
  • Initiate care plans and complete necessary assessments per healthcare guidelines.
  • Adhere to all departmental and organizational policies and procedures.
  • Perform additional duties as assigned.

Qualifications:

  • Education: High School Diploma/GED required; Associate's degree preferred.
  • Experience: Minimum of 2 years as an LVN in a healthcare environment, such as a medical clinic or acute care facility.
  • Language Skills: Proficient in English (written and verbal); bilingual candidates are preferred.
  • Licensure: Valid, unrestricted LVN license in the state; BLS and CPR certifications preferred.
  • Skills:
    • Proficient in computer use, including email and Microsoft Office applications.
    • Strong verbal and written communication skills, with the ability to convey information clearly.
    • Problem-solving skills with the ability to develop and implement effective solutions.
    • Ability to work independently and complete assignments within established guidelines.
    • Valid California driver's license and clean driving record; reliable transportation is necessary.

Physical Requirements: The role requires physical capabilities associated with office work and field service, including:

  • Regular use of office equipment such as computers and telephones.
  • Fieldwork providing services to members approximately 70% of the time.
  • Remote or office-based duties for about 30% of the time.
  • Occasional lifting of office equipment weighing up to 15 pounds.
  • Travel to various meetings and locations as necessary.
