At NewYork-Presbyterian, we're committed to delivering exceptional patient care and fostering a culture of respect, diversity, and inclusion. As a Care Manager RN Pediatric Oncology, you'll play a vital role in ensuring the continuity of care for our patients and their families.

In this critical position, you'll be responsible for consulting with patients to determine their needs, developing personalized care plans, coordinating patient-care services, and educating them about their condition. You'll work closely with the interdisciplinary team of social workers, physicians, physical therapists, occupational therapists, oncologists, and mid-level practitioners to provide comprehensive care.

You'll serve as a strong and knowledgeable liaison between the care team and patients, ensuring that their needs are met and their care is optimized. Your expertise will help prevent hospital readmission, anticipate outpatient service needs, and set appointments and consultations prior to discharge.

Key Responsibilities:

  • Consult with patients to determine their needs and develop personalized care plans
  • Coordinate patient-care services and educate patients about their condition
  • Work closely with the interdisciplinary care team to provide comprehensive care
  • Prevent hospital readmission and anticipate outpatient service needs
  • Set appointments and consultations prior to discharge

Requirements:

  • Bachelor's degree in Nursing (BSN)
  • Current NYS Registered Nurse (RN) licensure
  • 3-5 years of healthcare experience in an acute care setting
  • Superior communication, problem-solving, and interpersonal skills
