POSITION SUMMARY:

The Licensed Practical Nurse collaborates with a multidisciplinary team to provide care for residents with diverse health challenges. Key responsibilities include liaising with physicians and Registered Nurses regarding patient requirements, administering medications and IVs, and utilizing medical devices to monitor vital signs.

ESSENTIAL JOB FUNCTIONS:

  • Embrace and promote the mission, vision, and values of CIEL Senior Living.
  • Engage in comprehensive shift reports to ensure continuity of care.
  • Maintain accurate documentation in accordance with organizational standards to meet residents' needs.
  • Oversee the work of Resident Assistants during nursing shifts.
  • Participate in narcotic inventory counts at shift changes, ensuring precise documentation.
  • Administer medications following the established protocols for safe medication practices.
  • Conduct weekly audits of medication carts as assigned.
  • Assist in the reordering of medications as necessary.
  • Ensure the medication room is clean and organized.
  • Facilitate timely communication with pharmacies regarding new medication orders.
  • Notify families about the need for mail-order medication refills.
  • Coordinate with physician offices to manage medication orders effectively.
  • Adhere to Physician Order Protocols for accurate order processing.
  • Document any changes in residents' conditions in accordance with policies.
  • Assist in conducting chart audits and maintaining compliance.
  • Support the development and upkeep of resident care plans.
  • Accurately document incident reports in a timely manner.
  • Update Do Not Resuscitate (DNR) orders as required.
  • Collaborate with the Health and Wellness Director to maintain resident care files.
  • Participate in health initiatives such as flu clinics and TB testing.
  • Engage in community wellness programs as assigned.
  • Record residents' vital signs, including temperature, blood pressure, pulse, and respiration.
  • Monitor residents for reactions to medications and report any unexpected responses.
  • Provide counseling to residents to help address social or personal issues.
  • Schedule medical appointments based on residents' needs.
  • Respond promptly to residents' call lights to ensure their needs are met.
  • Attend required training sessions and conferences to stay updated in the nursing field.
  • Complete all mandated training as per state regulations annually.
  • Ensure all caregiver responsibilities are fulfilled and documented appropriately.
  • Address concerns from associates, families, and residents to foster satisfaction and communication.
  • Be knowledgeable about fire evacuation and disaster policies, participating in drills as required.
  • Maintain confidentiality regarding all personal and health information of residents and staff.
  • Demonstrate a strong understanding of compliance and ethics policies.
  • Complete all necessary compliance, HIPAA, and regulatory training as mandated.
  • Participate in all required staff meetings.
  • Perform additional duties as assigned to support the community's success and enhance resident care.

REQUIREMENTS:

EXPERIENCE, QUALIFICATIONS & SKILLS:

  • Education required for obtaining a state nursing license.
  • Current LPN licensure in good standing is mandatory.
  • Preferred experience of one to two years in assisted living settings.
  • CPR certification is essential.
  • Strong command of the English language for effective communication and documentation.

PHYSICAL DEMANDS:

  • Ability to walk and stand for extended periods.
  • Capable of performing duties in both indoor and outdoor environments.
  • Occasional sitting is required.
  • Regular lifting of weights ranging from 0-10 pounds, with frequent lifting of 11-50 pounds, and occasional lifting of up to 100 pounds.
  • Frequent carrying of weights up to 50 pounds.
  • Ability to push weights as required.
  • Frequent climbing of stairs, with elevator access available.
  • Frequent bending and manipulation of weights, with the ability to lift up to 50 pounds.
  • Frequent squatting to reach lower areas.
  • Occasional twisting or rotating movements.
  • Frequent reaching, with occasional overhead reaching required.
