POSITION SUMMARY:

The Licensed Practical Nurse collaborates with a team of healthcare professionals to provide care for patients facing various health challenges. Key responsibilities include liaising with physicians and Registered Nurses regarding patient needs, administering medications and IVs, and utilizing medical equipment to monitor vital signs.

ESSENTIAL JOB FUNCTIONS:

  • Uphold and promote the mission, vision, and values of CIEL Senior Living.
  • Participate in shift reports to ensure continuity of care.
  • Document care services in accordance with organizational standards to meet residents' needs.
  • Supervise Resident Assistants during nursing department assignments.
  • Conduct narcotic counts during shift changes, ensuring accurate inventory documentation.
  • Administer medications in compliance with established protocols.
  • Engage in regular audits of medication carts as assigned.
  • Assist in the reordering of medications as necessary.
  • Maintain the cleanliness and organization of the medication room.
  • Ensure timely faxing of new medication orders to the pharmacy.
  • Communicate with families regarding mail order medication reorders.
  • Coordinate with physician offices to obtain or modify orders for residents.
  • Adhere to Physician Order Protocol for processing orders accurately.
  • Document changes in residents' conditions as per policy.
  • Assist in conducting chart audits and maintaining accurate records.
  • Support the development and upkeep of resident service plans.
  • Accurately document incident reports in a timely manner.
  • Update Do Not Resuscitate (DNR) orders as required.
  • Assist the Health and Wellness Director in managing resident care files.
  • Participate in community wellness initiatives and health clinics.
  • Record residents' vital signs and report any unusual reactions to medications.
  • Provide counseling to residents on social or health-related issues.
  • Schedule medical appointments based on residents' needs.
  • Respond promptly to residents' call lights to address their needs.
  • Engage in ongoing training and professional development to stay current in nursing practices.
  • Complete all state-required training and documentation annually.
  • Ensure all caregiver responsibilities are fulfilled and documented appropriately.
  • Address concerns from associates, families, and residents to foster satisfaction and communication.
  • Be knowledgeable of fire evacuation and disaster policies, participating in drills as assigned.
  • Maintain confidentiality of all personal and health information.
  • Understand and comply with all ethics and compliance program policies.
  • Complete mandatory training related to compliance, HIPAA, and regulatory standards.
  • Attend all required staff meetings.
  • Perform other duties as assigned to support the community and enhance resident care.

REQUIREMENTS:

EXPERIENCE, QUALIFICATIONS & SKILLS:

  • Must possess a valid state nursing license.
  • Current LPN licensure in good standing is required.
  • Preferred experience of one to two years in assisted living settings.
  • CPR certification is mandatory.
  • Strong command of the English language for effective communication.

PHYSICAL DEMANDS:

  • Ability to walk and stand for extended periods.
  • Capable of performing tasks in both indoor and outdoor environments.
  • Occasional sitting is required.
  • Frequent lifting of weights ranging from 11 to 50 pounds.
  • Ability to climb stairs regularly.
  • Frequent bending and reaching required.
  • Occasional twisting and rotation of the body.
