Job Summary

We are seeking a highly skilled and compassionate Blended Case Manager to join our team at the Community Guidance Center. As a Blended Case Manager, you will play a critical role in providing outreach services to consumers, managing their caseload, and ensuring their well-being.

Key Responsibilities
  • Manage a caseload of 30 consumers, providing outreach services and support to meet their needs.
  • Develop and implement service plans in collaboration with consumers, focusing on their strengths and goals.
  • Link consumers with community resources and services, ensuring they have access to necessary support.
  • Monitor service delivery, addressing any issues or concerns that arise.
  • Provide crisis intervention and support, serving as the primary contact for consumers in crisis situations.
  • Document all interactions and services provided, maintaining accurate and thorough records.
  • Participate in regular supervision and training, staying up-to-date on best practices and Sanctuary Model principles.
Requirements
  • Bachelor's degree in a related field, such as social work, psychology, or sociology, or a registered nurse with relevant experience.
  • At least 5 years of experience in mental health direct care, case management, or a related field.
  • Act 33 & 34 clearances and FBI clearance, as well as Act 31 Mandated Reporter Training.
  • Excellent communication and interpersonal skills, with the ability to work effectively with consumers, staff, and community partners.
  • Strong organizational and time-management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • A valid driver's license and reliable vehicle, with the ability to travel to consumer appointments and meetings.
