About the Role

Gadsden Regional Medical Center is seeking a skilled Care Management Navigator to join our team. As a key member of our care management team, you will play a critical role in improving patient engagement and overall experience with acute and post-acute care services.

Key Responsibilities

  • Coordinate healthcare services from acute and post-acute care settings to ensure effective care transitions and communication with patients' primary care physicians.
  • Engage patients in their overall healthcare management, promoting self-advocacy and empowerment.
  • Collaborate with healthcare professionals to identify and address patient needs, ensuring timely and appropriate interventions.
  • Develop and implement care plans that address patients' physical, emotional, and social needs.
  • Monitor and evaluate patient outcomes, making adjustments to care plans as needed.
  • Provide education and support to patients and their families on healthcare services, benefits, and resources.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.

Requirements

  • Bachelor's degree in a healthcare-related field (e.g., nursing, social work, health sciences).
  • Minimum of two years of clinical or healthcare experience, preferably in care management or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple projects.
  • Proficiency in electronic health records and other healthcare software systems.
  • Knowledge of healthcare regulations, policies, and procedures, including managed care and value-based healthcare.
