Job Summary

Catholic Charities is seeking a skilled Housing Support Case Manager to join our team. As a key member of our staff, you will provide supportive and case management services to homeless families, helping them obtain and maintain permanent housing and stability.

Key Responsibilities
  • Provide case management services to families enrolled in the SF HOME Program, including housing search assistance, crisis intervention, and community and support services.
  • Develop and monitor individualized case plans for each family, with goals, objectives, and timelines.
  • Provide ongoing support services, including practical assistance, education, and advocacy to help families achieve stability and life management skills.
  • Act as a liaison between landlords and program participants to ensure housing stability.
  • Maintain accurate and timely data entry, and submit case records in a timely manner.
  • Participate in staff meetings, client peer reviews, and other training as required.
Requirements
  • BA degree and one year of experience, or AA degree and three years of related experience.
  • Previous experience locating and maintaining affordable family housing.
  • Experience working with families in crisis.
  • Strong coordination skills, knowledge of community resources, and ability to assess emergency situations.
  • Ability to communicate clearly in both verbal and written forms, and to prioritize tasks with strong organizational skills.
