Estimated Salary: $60,000 - $80,000 per year, depending on location and experience.

Job Description:

We are seeking a dedicated Patient Care Coordinator to join our team. As a key member of our treatment team, you will provide quality care through developing, implementing, managing, and evaluating patient care plans.

You will work closely with patients, families, and healthcare professionals to ensure optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.

Key Responsibilities:

  • Coordinating continuum of care activities for assigned patients
  • Developing and implementing individualized patient care plans
  • Conducting assessments, treatment planning, and crisis intervention services
  • Providing training and facilitating linkages for community resources
  • Monitoring service delivery and obtaining necessary services for patients

Requirements:

  • High school diploma or equivalent required
  • Training or education in mental health or substance use disorder competencies preferred
  • Basic competencies in mental health or substance use disorder and recovery skills
  • Valid/Current Driver's License required
