Job Summary

We are seeking a highly skilled RN Care Manager to join our team. As a RN Care Manager, you will play a critical role in supporting patient care for individuals with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). You will work closely with the care team to craft personalized care plans, conduct regular assessments, and provide proactive outreach to patients and clinics.

Key Responsibilities
  • Develop and implement comprehensive care plans that address the clinical and non-clinical needs of patients
  • Conduct regular assessments to detect undisclosed medical or social issues and changes in the home environment
  • Initiate proactive outreach to patients and clinics to evaluate their status and offer assistance
  • Manage a diverse patient panel and communicate updates to the care team and family members
  • Act as a liaison with dialysis facilities and maintain accurate program software records
  • Generate and evaluate reports using Strive data platforms to identify trends and areas requiring focus
Requirements
  • Associate's degree from an accredited school of nursing
  • Current Registered Nurse (RN) license in the state of practice
  • 3+ years combined of related education, experience, or certification
  • Current BLS or CPR Certification required
  • Efficient and reliable transportation allowing for the ability to commute to patient's homes
Preferred Qualifications
  • 3+ years experience in nephrology, ER, ICU, Cardiac, or Ambulatory Complex case management nursing
  • Certified Case Manager (CCM) certification preferred
  • Proficiency in computer skills including Microsoft 365 Suite (word, outlook, excel)
