We are seeking a skilled and compassionate Registered Professional Nurse to join our Home Health team at Providence Medford Medical Center. As a Home Health Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.
Key Responsibilities- Assess patient needs and develop individualized care plans to meet their unique requirements.
- Provide direct and indirect care to patients, including wound care, medication management, and education on self-care techniques.
- Collaborate with the healthcare team to coordinate patient care activities and ensure seamless transitions between care settings.
- Document patient progress and outcomes, maintaining accurate and comprehensive records.
- Participate in ongoing education and professional development to stay current with best practices and industry advancements.
- Current Oregon Registered Nurse License.
- 1 year of medical/surgical experience in an acute care setting, with a focus on home health care.
- National Provider BLS - American Heart Association certification.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
