RN Home Health Nurse

As a Home Health Registered Nurse with Providence, you will play a vital role in delivering high-quality patient care in the comfort of patients' homes. You will work collaboratively with a multidisciplinary team to provide comprehensive care that addresses the unique needs of each patient.
Key Responsibilities- Assess patients' needs and develop individualized care plans to meet their physical, emotional, and social needs.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings.
- Conduct home visits to assess patients' environments and provide education on self-care and disease management.
- Document patient information and progress in a timely and accurate manner.
- Current Oregon Registered Nurse License.
- 1 year of medical/surgical experience in an acute care setting required for Home Health RN.
- 2 years of Pediatric experience in an acute care setting for RN caring for pediatric patients.
