We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Providence Medford Medical Center. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.
Key Responsibilities- Assess patient needs and develop individualized care plans to meet their unique requirements.
- Provide direct patient care, including wound care, medication management, and education on self-care techniques.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and communicate effectively with families and caregivers.
- Current Oregon Registered Nurse License.
- 1 year of Medical/Surgical experience in an acute care setting required for Home Health RN.
- 2 years of Pediatric experience in an acute care setting for RN caring for pediatric patients.
