Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Signature Healthcare at Home. As a LPN, you will provide direct patient care under the supervision of a registered nurse, following a comprehensive assessment and the plan of care.

The ideal candidate will have a strong foundation in nursing principles and practices, with a focus on delivering high-quality patient care in a home health setting.

  • Provide direct patient care as defined in State Nurse Practice Act.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Evaluate patient's response to treatments/medications.
  • Participate in personal, professional growth and development.
  • Complete mandatory education requirements annually to maintain competence.
  • Attend required staff meetings and patient care coordination activities.
  • Perform other duties as assigned by the Registered Nurse or Clinical Manager.
Responsibilities

The Licensed Practical Nurse will be responsible for providing direct patient care, including following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

The LPN will also be responsible for evaluating patient's response to treatments/medications, participating in personal, professional growth and development, and completing mandatory education requirements annually to maintain competence.

Qualifications

To be successful in this role, the candidate will need to have a strong foundation in nursing principles and practices, with a focus on delivering high-quality patient care in a home health setting.

The ideal candidate will have a graduate degree from an accredited practical nurse or vocational nursing program, with one to two years of nursing experience preferred. The candidate will also need to be currently licensed as an LPN/LVN in the State(s) of planned practice.

The candidate will need to demonstrate good verbal and written communication, and organization skills, with the ability to work effectively in a team environment.

The candidate will also need to possess and maintain current CPR certification, with knowledge of geriatric population and their common needs and concerns.

The candidate will need to be able to lift, position and/or transfer patients, with the ability to lift supplies and equipment, and possess visual acuity and hearing to perform required nursing skills.
