About the Role:

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team in providing comprehensive health assessments to at-risk individuals in their homes.

Key Responsibilities:

  • Conduct thorough assessments of patients' medical history, health status, and social environment.
  • Develop and implement care plans to address identified health risks and needs.
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their caregivers to promote healthy behaviors and disease management.

Requirements:

  • Master's degree in Nursing with a focus on Family Nurse Practitioner or related field.
  • Current RN and NP licensure in the state of practice, with prescriptive authority.
  • Board certification in a Matrix-approved specialty.
  • Minimum 1 year of experience as a Nurse Practitioner, with a strong understanding of adult and geriatric care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
