About the RoleThe Clinical Transition Nurse will be responsible for providing high-quality patient care in a fast-paced and dynamic environment. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care
- Collaborate with the patient care team to achieve quality patient outcomes
- Communicate effectively with patients, families, and healthcare team members
- Participate in unit-based quality improvement initiatives
- Stay current with evidence-based practice and research
To be successful in this role, you will need:
- A graduate degree in nursing from an accredited program
- At least one year of clinical experience as a licensed Registered Nurse in a children's hospital or similar setting
- Current licensure as a Registered Nurse in the state of Texas
