Job Summary

We are seeking a skilled Registered Nurse to join our team as a Clinical Transition Nurse. This role is responsible for providing exceptional patient care and ensuring a seamless transition for patients coming in and out of acute care hospital settings.

The ideal candidate will have a strong nursing background and excellent communication skills. They will be responsible for assessing patients, developing care plans, and educating patients and families on their medical conditions and treatment plans.

Requirements
  • Registered Nurse license in Texas or Compact State
  • 1-2 years of experience in a hospital setting
