Job Description

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Yale-New Haven Health.

The successful candidate will be responsible for providing exceptional care to patients in the psychiatric emergency department, working collaboratively with a multidisciplinary team to achieve optimal outcomes.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical and psychosocial conditions, identifying areas of concern and developing personalized plans of care.
  • Implement and coordinate treatment plans in collaboration with healthcare teams, ensuring seamless delivery of patient-centered care.
  • Evaluate the effectiveness of treatment plans, making adjustments as needed to ensure positive patient outcomes.
  • Provide education and support to patients and their families, empowering them to manage their health effectively.
  • Serve as a preceptor to new staff members, fostering a culture of knowledge sharing and professional development.
  • Maintain a safe environment for patients and colleagues, adhering to standards of professional behavior and organizational policies.
Qualifications:
  • Graduate of an accredited nursing program, with a Bachelor's degree preferred.
  • Valid State of Connecticut Registered Nurse license, with Basic Life Support (BLS) certification required.
  • Experience in mental health or emergency department settings is highly desirable, with hospital and unit-based orientation provided.
Estimated Salary:

$83,000 - $115,000 per year, depending on experience.
