Job Summary

The Transplant Coordinator delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care.

Key Responsibilities
  • Delivers and documents direct, age-specific care to patients based on medical orders and a multidisciplinary plan of care.
  • Safely administers prescribed medications, identifying incompatibilities and/or interactions.
  • Manifests a high level of expertise in the assessment, coordination, and care of complex patients, families, or communities to actual or potential health problems.
  • Prevents illness and injury, maintains wellness, and provides comfort.
  • Provides direction, education, and leadership to the healthcare team.
Requirements
  • Current RN licensure in Virginia or eligibility.
  • AHA BLS HCP Certification or equivalent.
  • Minimum of three years of nursing experience in transplant, critical care, or medical-surgical nursing.
  • Two years of progressive leadership experience.
  • Demonstrated clinical expertise for the population served with a minimum of one year in the specialty area.
Preferred Qualifications
  • Five years of clinical experience in the transplant field.
  • Academic healthcare experience.
  • Advanced practice nursing role or clinical leadership experience.
Education/Training
  • Baccalaureate degree in nursing from an accredited school of nursing.
  • Masters degree in nursing certification in transplantation.
Independent Action

Follows documented physician/licensed independent provider orders within the boundaries of the Nurse Practice Act and established standards, policies, and practice guidelines.

Organizes and plans work with input from the patient/family with specific outcomes and demonstrated use of sound clinical judgment and resources.

Handles all day-to-day operations of the role independently with minimal assistance.
