Job Summary

The Triage Nurse plays a vital role in the Hospice interdisciplinary team, delivering exceptional care through triage calls after hours and on holidays. This position is responsible for ensuring timely and efficient completion of tasks, prioritizing patient needs, and meeting productivity requirements.

Key Responsibilities
  • Task Management: Complete POC tasks within 30 minutes of request, including data entry of referrals, eligibility, scheduling, quick holds, approval of orders and referrals.
  • Escalation: Escalate requests to manager when a resolution is not identified by user, with escalation to help desk only after discussing with manager.
  • Patient Prioritization: Prioritize incoming tasks according to patient needs.
  • Productivity: Meet minimum productivity requirements of 100 tasks per day on weekends and holidays, 50 tasks per day on weekdays.
  • Team Collaboration: Participate in department staff meetings and educational sessions.
  • Additional Duties: Perform other duties as assigned by director.
Qualifications
  • Education: Associate Degree or certification equivalent.
  • Experience: 3 years of experience.
  • Licensure and Certification: Current RN Licensure for all applicable states assigned prior to accepting call for agencies.
  • Skills: Proficient in HCHB, excellent customer service and communication skills, strong computer skills, and ability to type.
  • Problem-Solving: Strong problem-solving and analytic skills.
  • Internet Requirement: High-speed internet connection (not a satellite connection).
