Role Overview
The Clinical Care Coordinator plays a pivotal role in managing interdisciplinary communications and facilitating care throughout a patient's hospital stay. This position involves evaluating the appropriate use of resources while respecting the patient's autonomy. Responsibilities include:
- Assessing patient hospitalizations and recommending suitable levels of care that align with medically necessary criteria.
- Ensuring accurate documentation of care orders and other essential records in the medical file.
- Engaging in discussions regarding alternative care options with attending physicians and emergency department staff.
- Acting as a liaison among patients, caregivers, multidisciplinary team members, and third-party payers.
- Collaborating with the care team to create and execute discharge plans tailored to individual patient needs.
Education:
- Required: Registered nurse diploma or equivalent.
- Preferred: Bachelor's degree in nursing.
- Required: None.
- Preferred: 3 years of experience in a hospital setting focusing on discharge planning, case management, or utilization review.
- Required: Current registered nurse license in the state of practice and Basic Life Support (BLS) certification from the American Heart Association.
- Preferred: Certification in Case Management (CCM) or Accredited Case Manager (ACM).
- Proficient in computer systems, software, and web applications.
- Strong verbal and written communication skills, capable of presenting information clearly to diverse audiences.
- Excellent interpersonal skills.
- Flexibility to work various schedules, including weekends and holidays.
- Conducting pre-admission, admission, and ongoing stay reviews in accordance with CMS and payer guidelines.
- Embodying the values of service excellence, teamwork, and integrity while pursuing personal and professional growth.
- Staying informed about participation conditions related to utilization review and discharge planning.
- Maintaining open lines of communication with all relevant parties, acting as a bridge between physicians and care teams.
- Overseeing the management of functional and financial outcomes, including the coordination of discharge plans during acute care.
- Adapting interactions to respect the unique needs of each patient population.
- Performing additional related duties as necessary.
