**Overview:**

The Commonwealth Care Alliance is seeking an experienced Registered Nurse to join our Care Planning Unit. As a valued member of our team, you will play a critical role in ensuring the highest quality care for our members, while driving innovative solutions to improve health outcomes and reduce costs.

About the Position:

  • Utilize your expertise in community health and care management to assess the needs of our members, develop individualized care plans, and collaborate with our interdisciplinary team.
  • Work closely with our nurses, social workers, and other healthcare professionals to ensure seamless communication and coordination of care.
  • Participate in regular case reviews and quality improvement initiatives to continually enhance our services and outcomes.

Responsibilities:

  • Conduct in-person assessments of our members, gathering essential information to inform their care plans.
  • Develop and implement comprehensive care plans that address the unique needs and goals of each member.
  • Communicate regularly with our Care Planning Team, Nurses, and other healthcare professionals to ensure effective care coordination.
  • Monitor and evaluate the effectiveness of our care plans, making adjustments as needed to optimize outcomes.

Requirements:

  • Demonstrated experience as a Registered Nurse with a strong background in community health and care management.
  • Excellent assessment and clinical decision-making skills, with the ability to navigate complex patient scenarios.
  • Proven ability to work effectively in a fast-paced environment, prioritizing tasks and managing competing demands.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse stakeholders, including patients, families, and healthcare teams.

Compensation and Benefits:

  • A competitive salary range of $80,000 - $110,000 per year, depending on experience.
