About the Opportunity

We are seeking a dedicated and compassionate Assessment and Care Planning Specialist to join our team at Commonwealth Care Alliance. As a key member of our care planning unit, you will play a critical role in developing and implementing care plans that promote optimal health outcomes for our members.

In this role, you will work closely with interdisciplinary teams to assess member needs, develop care plans, and provide clinical care and support. You will also participate in ongoing quality improvement initiatives to enhance the delivery of care services.

About the Job

Job Summary: The Assessment and Care Planning Specialist is responsible for assessing member needs, developing and implementing care plans, and providing clinical care and support to individuals with complex needs.

Key Responsibilities:

  • Assess member needs and develop comprehensive care plans
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Provide clinical care and support to members with complex needs
  • Participate in ongoing quality improvement initiatives to enhance the delivery of care services
  • Manage assessment activities through calendar and track status of completion

Requirements:

  • Associate's degree in Nursing required; Bachelor's degree preferred
  • Active RN licensure in Commonwealth of Massachusetts
  • Minimum 2 years of experience in acute/medical surgical settings or home care/community settings
  • Experience with electronic medical records (EMRs) preferred

Salary: $65,000 - $80,000 per year
