We are seeking a dedicated and compassionate Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will be responsible for providing high-quality care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
Key Responsibilities- Build Meaningful Relationships: Develop strong relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
- Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in the level of care and service as needed.
- Assist with Life Skills: Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
- Participate in ISP Development: Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Ensure Resident Safety: Ensure the safety and well-being of residents during the shift, knowing their whereabouts and physically checking on them as needed.
- Partner with Community Team: Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
- Provide Excellent Customer Service: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
- Dedication to Serving Seniors: Demonstrate a passion for serving seniors with excellent customer service skills.
- Positive Attitude and Flexibility: Possess a positive attitude and the flexibility to perform various duties in service to residents.
- High School Diploma or Equivalent: Hold a high school diploma or equivalent, and may be required to maintain certifications in certain states.
- Previous Experience Working with Seniors: Previous experience working with seniors is preferred.
- Ability to Make Decisions: Possess the ability to make choices, decisions, and act in the best interest of residents.
