Job Title: Client Care Manager

We are seeking a highly skilled and compassionate Client Care Manager to join our team at Homewatch CareGivers of Eatontown. As a Client Care Manager, you will be responsible for providing exceptional care and support to our clients, while also building strong relationships with their families and caregivers.

Key Responsibilities:
  • Handle initial leads and validate client needs
  • Schedule in-person visits with potential clients to assess their needs
  • Responsible for sending and executing service agreements
  • Build rapport with assigned clients, family members, and caregivers
  • Act as primary contact for clients, communicating schedule changes and resolving issues in real-time
  • Communicate changes to client schedules or conditions to the client service team
  • Get to know caregivers and make recommendations for matching
  • Coordinate Registered Nurse visits and the first day of service
  • Participate in weekly client service meetings
  • Be part of the on-call weekend rotation and participate in caregiver interviews
Requirements:
  • Minimum: Certified Nursing Aide (CNA) preferred or Licensed Practicing Nurse (LPN), Licensed Master of Social Work (MSW), Licensed Clinical of Social Work (LCSW)
  • At least 2 years of clinical experience, must be fully vaccinated against COVID-19
  • Strong oral and written communication skills, articulate, courteous, and friendly
  • Attention to detail and ability to work in a fast-paced environment
  • Intermediate computer skills in Microsoft Office products and care management software
