Key Responsibilities:
  • Handle initial leads and schedule in-person visits with potential clients to assess their needs.
  • Responsible for sending and executing service agreements.
  • Build rapport with assigned clients, family members, and caregivers as the primary point of contact.
  • Communicate schedule changes and resolve client issues in real-time.
  • Coordinate Registered Nurse visits and the first day of service.
  • Participate in weekly client service meetings and be part of the on-call weekend rotation.
  • Play a role in interviewing caregivers.
Requirements:
  • Certified Nursing Aide (CNA) or Licensed Practicing Nurse (LPN), Licensed Master of Social Work (MSW), or Licensed Clinical Social Worker (LCSW) preferred.
  • At least 2 years of clinical experience and must be fully vaccinated against COVID-19.
  • Strong oral and written communication skills, with a winning smile and a can-do attitude.
  • Intermediate computer skills in Microsoft Office products and care management software.
