Our clientele primarily resides in the seven-county metropolitan areas, and we collaborate with various lead agencies.
Key Responsibilities:
As a Case Manager at Pinnacle Services, you will engage with clients utilizing a combination of CADI and DD waivers. The individuals you support may face developmental disabilities, serious mental health challenges, substance use disorders, physical disabilities, or age-related issues.
Your responsibilities will include:
- Evaluating the aspects of the client's life where support is necessary.
- Providing and coordinating resources tailored to the client's needs.
- Overseeing service delivery and collaborating with providers to ensure client requirements are fulfilled.
- Developing comprehensive service plans.
- Participating in regular meetings with clients and their interdisciplinary teams.
- Documenting client-related activities to generate billable units.
Compensation:
Non-LSW salary: $49,000 per year, based on a 40-hour work week.
LSW salary: $51,600 per year, based on a 40-hour work week.

Qualifications:
Essential Requirements:
- A degree from an accredited four-year institution in social work, psychology, sociology, or a closely related field, or
- A degree in any field combined with one year of experience as a social worker, case manager, or care coordinator in a social service agency.
- At least one year of experience in the education or treatment of individuals with developmental disabilities or related conditions, or completion of a course specifically addressing developmental disabilities.
- A valid driver's license and a satisfactory driving record as per Pinnacle Services policy, along with a vehicle that has current insurance.
- Successful completion of local and/or state background checks.
Preferred Qualifications:
- Possession of LSW or higher licensure from the Board of Social Work.
