- Conduct in-home assessments, evaluating past medical history, reviewing systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
- Identify diagnoses for care management and active medical management.
- Communicate findings to inform member's primary care physician of potential gaps in care.
- Active NP or PA license; ability to obtain by start date.
- Active national NP or PA certification; ability to obtain national certification in state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs.
