DescriptionWage Scale: $22.06-$28.94JOB SUMMARY: The Care Coordinator II will be the program coordinator for referrals to Wrap-Around services, receive all Wrap referrals, and assign to self or other Position. Provides Wrap-Around Team planning, coordination, and administration of Wrap-Around services drawing on the strengths of individuals and families served; work effectively with families, community partners, and mental health team, maintain required record, and case management as assigned.ESSENTIAL FUNCTIONS: Coordinates with Child & Family team for referrals to Wrap-Around program Coordinates and facilitates wraparound team meetings which would include child (if appropriate), family members, child/family supports, school representative, and other community partners (i.e. child welfare caseworkers, juvenile PO's) when appropriate Assesses child, youth, and family strengths and needs Coordinates with service providers for implementation of wraparound planning, including development of an individualized care coordination plan Coordinates follow-up wraparound team meetings according to timelines of each specific phase Develops with the Child & Family Team a coordinated proactive crisis/safety plan that addresses immediate and ongoing needs Fosters and maintains ongoing relationships with and communication between Child & Family Team members (including, but not limited to, the family, providers, agency representatives, natural supports, and peer delivered services). Looks for opportunities to build and expand natural and informal supports Monitors implementation of the Plan of Care and action steps of team members Provides case management services for each child/family involved in Wrap-around services; assist the mental health child/family clinicians with case management client services as assigned Undergoes any required training to perform the state-required CANS/Child Assessment of Needs; become proficient in entering CANS and Wrap-Around requirements into state and/or regulatory agency databases. Completes documentation in client charts within agency stated timelines Models and encourages direct, professional communication among staff members and community partners; facilitate development of positive staff/community support network Demonstrates awareness and sensitivity in the delivery of consumer services as it relates to cultural and socioeconomic characteristics of the population served Participates in any training required for Wrap-Around Coordinators and stay abreast of new administrative rules/statues pertaining to this program. Other duties as assigned by immediate supervisor or Program DirectorORGANIZATIONAL RESPONSIBILITIES:Performs work in alignment with the BestCare's mission, vision, values and professional code of ethics Supports the organization's commitment to diversity, equity, and inclusion by fostering a culture of open-mindedness, cultural awareness, compassion, and respect for all individuals; Strives to meet Program/Department goals and supports the organization's strategic goals; Adheres to the organization's Code of Conduct, Business Ethics, Employee Handbook, and all other relevant policies, procedures, and other compliance needs; Understands and maintains professionalism and confidentiality per HIPAA, 42 CFR, and Oregon Statutes; Attends required program/staff meetings and completes assigned training timely and satisfactorily; Ensures that any required certifications and/or licenses are kept current and renewed timely; Works independently as well as participates as a positive, collaborative team member;Acts independently in the community and promotes a positive image of Crook County, the Intellectual/Developmental Disabilities Program, and BestCare Treatment Services; Performs other organizational duties as needed;QUALIFICATIONS:EDUCATION AND/OR EXPERIENCE REQUIREMENTS: Must have a combined 3 years of education/equivalent work experience; such as an Associate's degree in a behavioral science, social science, or closely related field plus 1 year of human services-related experience or High School Diploma (or equivalent) plus 5 years' equivalent of human services-related experience Experience in the counseling/human services arena, preferably in community mental health and with children and families, coordinating teams, strength-based planning For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hireLICENSES AND CERTIFICATIONS: QMHA-I certification or ability and commitment to obtain within 6 months Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization's auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations) PREFERRED: Bachelor's degree in social work, human services, counseling, psychology, or social services preferred Bilingual in English/Spanish a plusREQUIRED COMPETENCIES: Must have demonstrated competency or ability to attain competency for each of the following within a reasonable period: Knowledge of mental health and addiction problems, as well as community resources; Ability to communicate clearly, concisely, and effectively (both orally and in writing); Must have cultural sensitivity and a fundamental respect toward diverse populations and ability to work well with parents and children of diverse backgrounds Must demonstrate ability to comply with program policies and procedures and all regulations for client case management and recordkeeping Must demonstrate ability to record information in assigned client files, consistent with program policies and procedures and applicable OAR's; Must have demonstrated skills in developing cooperative and collaborative efforts with providers, families, advocates, and other community partners; Empathy, compassion, and willingness to work with people with disabilities and/or families with major challenges such as low levels of parenting skills, poverty, etc. Proficiency in use of Microsoft applications (Office 365, Word, Excel, Outlook, One-Note, Teams, PowerPoint), databases, virtual meeting platforms, internet, and ability to learn new or updated software; Proficiency using electronic medical record systems (EMR); Ability to maintain strict confidence as required by HIPAA, 42 CFR, and Oregon Statutes; Ability to build and maintain positive relationships; Strong interpersonal and customer service skills; Strong communication skills (oral and written); Strong organizational skills and attention to detail, accuracy, and follow-through; Excellent time management skills; Critical thinking skills and ability to function effectively and use good judgment in crisis situations; Ability to manage conflict resolution and anger/fear/hostility/violence of others appropriately and effectively; Ability to work independently as well as participating as a positive, collaborative team member.TOOLS AND EQUIPMENT USED: Standard office equipment including computer/laptop, copy/fax/scanning machine, printer, and telephone or multi-line phone.WORK ENVIRONMENT/PHYSICAL DEMANDS: The work environment is respectful, multi-cultural and inclusive, and the characteristics described above are representative of those an employee encounters while performing the essential functions of this job.
