We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care to patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities- Provide one-to-one patient care in a home environment
- Execute physicians' prescribed plans of care and document care provided in our system of record
- Administer prescribed medication, treatments, and therapies
- Patient assessments and coordination of care
- Health promotion, teaching, and training of family members
- Good standing license (LPN) in the state in which the clinician will practice
- At least 6 months of hands-on nursing experience
- Current CPR certification
- Willingness to provide in-home nursing care to infants, children, and adolescents
