Job Title: Pediatric Home Health Nurse

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team at Angels of Care Pediatric Home Health. As a Pediatric Home Health Nurse, you will provide high-quality, patient-centered care to children and young adults in the comfort of their own homes.

Job Summary:

As a Pediatric Home Health Nurse, you will be responsible for providing direct, one-on-one medical care to patients in their homes. This includes conducting patient assessments, administering prescribed medications and treatments, and coordinating care with other healthcare professionals.

Responsibilities:
  • Conduct ongoing patient assessments and develop individualized care plans
  • Administer prescribed medications and treatments as ordered by physicians
  • Coordinate care with other healthcare professionals, including physicians, therapists, and social workers
  • Educate patients and families on disease management and self-care techniques
  • Provide emotional support and advocacy for patients and families
Requirements:
  • Active RN or LPN/LVN license
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced, dynamic environment
