Home Care Medical Specialist

We are seeking a compassionate Nurse Practitioner to join our team at Abode Care Partners, a leading provider of integrated medical services caring for individuals from post hospitalization to home in various settings.
About the Role:- Deliver evidence-based medical care to seniors and patients with complex needs in their homes and communities.
- Plan, direct, and evaluate patient medical needs, examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
As a Home Care Medical Specialist, you will collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care. You will also ensure that patient medical records are kept up-to-date and provide referrals to other healthcare practitioners when necessary.
Requirements:- An active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- A Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
