Compassionate Nurse Practitioner for Home-Based Medical Services

We are seeking a skilled Nurse Practitioner to join our team at Abode Care Partners, providing high-quality medical care to seniors and patients with complex needs in home and community settings.
Job OverviewThis role involves delivering evidence-based medical services, collaborating with caregivers, and maintaining comprehensive patient records while ensuring compliance with regulations. The successful candidate will have excellent communication skills, strong knowledge of HIPAA and OSHA regulations, and experience working with electronic medical records.
Responsibilities:- Medical Services Delivery: Providing compassionate, evidence-based medical care to seniors and patients with complex needs, including chronic illness treatment and senior care.
- Patient Care Planning: Collaborating with caregivers and other healthcare professionals to develop comprehensive care plans, ensuring seamless transitions and coordinated care.
- Record Keeping: Maintaining accurate and up-to-date patient medical records, adhering to HIPAA and OSHA regulations.
- Clinical Decision Making: Utilizing clinical expertise to make informed decisions regarding patient care, ensuring the highest quality outcomes.
- Licensure: Active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, and Active state DEA licensure.
- Education: Master's degree in Nursing (MSN).
- Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Skills: Strong interpersonal communication skills – verbal and written, Excellent time management skills with accountability for practitioner's daily workflow, Proficiency in medical instruments and equipment, and Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
We offer a competitive salary range of $100,000 - $120,000 per year, based on performance and experience, along with a comprehensive benefits package, including weekdays and daytime shifts only, mileage reimbursement for certain roles, flexible schedule, employee referral program, professional licensure reimbursement, tuition reimbursement on specific programs, paid time off, 401(k) retirement savings plan, medical, dental and vision insurance, life insurance, Flexible Spending Account (FSA), and employee discounts.
