We are seeking a highly motivated and compassionate Nephrology Nurse Specialist to join our team. As a Nephrology Nurse Specialist, you will be responsible for providing high-quality patient care to individuals with end-stage renal disease (ESRD). Your expertise will be invaluable in helping patients manage their dialysis treatment and improve their overall quality of life.
Key Responsibilities:
- Provide comprehensive patient care to individuals with ESRD
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare teams to ensure seamless care transitions
- Conduct patient education and training on dialysis treatment and self-care
- Monitor patient progress and adjust care plans as needed
Requirements:
- Current RN license in the state of practice
- Minimum of 12 months of RN experience
- Basic computer skills and proficiency in MS Word and Outlook
