Nephrology Care Nurse

About This Role:
DaVita is seeking a Registered Nurse to join our team of healthcare professionals who deliver compassionate and high-quality patient care in a dynamic and supportive environment.
As a Registered Nurse on our team, you will be responsible for providing direct patient care, conducting assessments, and developing individualized care plans to meet the unique needs of each patient. You will work in collaboration with a multidisciplinary team to provide comprehensive care, including hemodialysis, peritoneal dialysis, and CRRT.

Responsibilities:
- Provide direct patient care: Conduct assessments, develop individualized care plans, and implement interventions to meet the unique needs of each patient.
- Collaborate with the multidisciplinary team: Work with physicians, nurse practitioners, and other healthcare professionals to provide comprehensive care and achieve optimal patient outcomes.
- Contribute to quality improvement initiatives: Participate in quality improvement activities, including data collection, analysis, and implementation of evidence-based practices to enhance patient care and outcomes.
- Develop and maintain strong relationships with patients and families: Build trust, provide emotional support, and educate patients and their families on their care plans and treatment options.
