Job Summary:

We're seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team. As a key member of our team, you'll provide high-quality, cost-effective care to our members, working collaboratively with physicians and other healthcare professionals to ensure the best possible outcomes.

Responsibilities:
  • Primary Care Delivery: Deliver comprehensive, patient-centered care to assigned members, managing both medical and behavioral conditions effectively.
  • Care Coordination: Assess the medical necessity of ancillary services, coordinate care transitions, and monitor member needs to ensure seamless care delivery.
  • Program Enhancement: Promote the CCM program in assigned facilities, develop positive customer relationships, and adapt to changing circumstances to drive program growth and success.
Requirements:
  • Certified Nurse Practitioner or Physician Assistant through a national board
  • Active and unrestricted license in the State of Connecticut
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire
  • Access to reliable transportation and ability to travel to client and/or patient sites
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
  • Availability to work 24 hours per month, with expectations of 16 hours during off-hours
Preferred Qualifications:
  • 1+ years of hands-on post-grad experience in Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient computer skills and ability to document medical information
