Position Summary:

Key Responsibilities:

  • Conduct comprehensive assessments of patients' health and maintain accurate documentation in electronic health records (EHR).
  • Perform relevant diagnostic tests and interpret results appropriately.
  • Ensure patients are informed of their test results.
  • Collaborate with patients to create individualized care plans, including alternative treatment options.
  • Emphasize preventive care and health maintenance for optimal health outcomes.
  • Establish and nurture therapeutic relationships with patients.
  • Maintain professionalism to ensure objectivity in patient care.
  • Utilize the NH formulary for medication prescriptions.
  • Complete patient documentation after each visit and order necessary diagnostic tests.
  • Engage with appropriate referral contacts as needed.
  • Adhere to infection control protocols while providing care.
  • Facilitate patient and staff education initiatives.
  • Follow management guidelines to ensure productivity.
  • Promote professional development by attending relevant clinical education sessions.
  • Consult with the Medical Director when necessary for patient care.
  • Foster teamwork and collaboration among staff members.
  • Participate in staff meetings and lead committees when appropriate.
  • Engage in quality improvement initiatives.
  • Uphold patient confidentiality and comply with HIPAA regulations.
  • Perform additional duties as required.

Qualifications:

  • Registered Nurse with a Master's degree and licensed as ARNP in New Hampshire.
  • Experience in Family Medicine as an ARNP, capable of caring for patients of all ages.
  • Basic Life Support Certification.
  • A minimum of five years of experience in an ambulatory care setting as an Independent Primary Care Provider.
  • Ability to work collaboratively as a team member with strong problem-solving skills.
  • Exceptional interpersonal skills.
  • Proven organizational abilities.
  • Demonstrated reliability in professional settings.

Knowledge, Skills, and Abilities:

  • Ability to effectively communicate detailed information verbally.
  • Proficient in expressing and exchanging ideas through spoken communication.
  • Skilled in interacting with diverse individuals across various backgrounds and skill levels.
  • Strong listening abilities.
  • Visual acuity for analyzing clinical data and patient information.
  • Excellent written and verbal communication skills, along with computer proficiency.

Work Schedule: 32 hours per week.
