Client Intake Specialist

POSITION SUMMARY
This role operates under the administrative guidance of the Program Director or their designated representative, focusing on delivering psychosocial, rehabilitation, and recovery-oriented support to our members.
KEY RESPONSIBILITIES
This is a direct service position within a program framework. The incumbent is tasked with creating and executing individualized treatment plans aimed at addressing immediate concerns while facilitating the member's transition back to less intensive community environments at the earliest opportunity. Additional assistance in medication management, housing, counseling, and advocacy will be provided as necessary.
PRIMARY DUTIES AND TASKS
- Establishes and nurtures collaborative relationships with referring hospitals and outpatient service providers.
- Coordinates intake processes with Crisis Residential Programs, the Intake Team, and all referral sources.
- Conducts community interviews and formulates recommendations for program admissions.
- Completes necessary intake documentation with clients upon their entry into the program.
- Ensures all required documentation is received and processed from referral sources prior to client admission, including medication orders.
- Welcomes clients, introducing them to on-duty staff, assigned service coordinators, and other clients, while providing a facility tour and ensuring they are settled into appropriate accommodations. Collaborates with program leadership to address specific client needs and risk factors, such as medical issues and dietary requirements.
- Participates in ongoing training and professional development workshops.
- Evaluates clients for potential involuntary hospitalization and completes requisite forms as needed.
- Provides immediate intervention during crises to stabilize situations and notify necessary parties for resolution.
- Delivers on-the-spot counseling that aligns with the program's philosophy and supports client needs.
- Facilitates psychosocial groups, treatment planning, and discharge planning initiatives.
- Ensures the safety, health, and overall well-being of program members.
- Completes assigned paperwork in a timely and efficient manner.
- Offers support to colleagues as required.
- Represents the program at community meetings as assigned.
WORK SCHEDULE
Monday - Friday, 8:00 AM - 4:30 PM
QUALIFICATIONS
Education and Experience:
A typical pathway to acquiring the necessary knowledge, skills, and abilities includes earning a master's degree in Social Work or a related field from an accredited institution, along with valid registration with the Board of Behavioral Sciences (BBS).
Licenses and Certifications:
- Valid registration with the Board of Behavioral Sciences (BBS)
- California driver's license and current vehicle insurance/registration if driving is required.
- Reliable transportation capable of passing vehicle safety inspections if over five years old, excluding two-wheeled transport modes.
