We are seeking a highly skilled and experienced Long Term Care Pharmacist to join our team at A-Line Staffing Solutions.
Job Summary:The successful candidate will be responsible for evaluating physician medication orders, making recommendations for therapy changes, and maintaining patient/resident databases. They will also ensure the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
Key Responsibilities:- Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
- Make recommendations for therapy changes to the physician and facility staff as appropriate.
- Maintain patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other members of the healthcare team and patients/residents and their family members in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
- Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and PharMerica policy.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
- Colorado Pharmacist License required.
Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance. A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.
