Job Summary

We are seeking a skilled Care Navigator to join our team. As a Care Navigator, you will be responsible for developing and maintaining relationships with healthcare practitioners, physician practice offices, and facilities to promote referrals to Covenant Care.

Key Responsibilities
  • Develop and implement referral strategies to increase admissions to Covenant Care
  • Build and maintain relationships with key stakeholders, including healthcare practitioners and facility administrators
  • Provide education and training to team members on marketing and sales objectives and tactics
  • Stay up-to-date on laws and regulations related to marketing and sales in home health
  • Collaborate with the operations team to ensure seamless service delivery
Requirements
  • 2+ years of experience in healthcare sales or marketing
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in CRM software
